Loam Health is a nurse-led coaching practice focused on metabolic health, with an emphasis on A1c, prediabetes, insulin resistance, and long-term risk reduction for conditions like type 2 diabetes.

The work I do here bridges the gap between understanding what supports your health, and being able to consistently apply it in real life. This includes areas like nutrition, movement, sleep, stress, and medication support when relevant.

I work with adults navigating insulin resistance and metabolic health concerns, including those using GLP-1 medications who want help building sustainable habits alongside treatment.

That gap is not a reflection of discipline or capability. It reflects the complexity of applying health information inside the reality of work schedules, fatigue, stress, shifting routines, and competing priorities. Loam is focused on helping you bridge that gap in a way that is realistic and maintainable.

My work is grounded in clinical nursing experience, physiology, and behavior change science, with a strong focus on real-world implementation—health outcomes are shaped not just by knowledge, but by what is sustainable in your daily life.

Who’s Behind Loam Health

I’m Sarah Kilgallen, a registered nurse and board-certified nurse health coach (NC-BC). My clinical background includes bedside nursing, outpatient care, and patient education across a range of settings, along with continued training in behavior change and habit science.

Across my work in healthcare, I kept seeing the same pattern: people who understood their health but struggled to consistently follow through once life became busy, stressful, or unpredictable.

Loam Health grew from that gap.

Expect in an Intro Session

An introductory session is a structured conversation designed to understand your metabolic health context, and desired outcomes in a practical and individualized way.

We touch on your health history, current metabolic concerns, and relevant health information. From there, we identify what meaningful change would look like for you—this is where you get to dream big!

If it feels like a good fit, I’ll walk you through options for ongoing support.

I offer structured coaching programs in 12- and 16-week formats focused on implementation, habit formation, and real-world consistency.

What’s The Difference Between
Loam Health’s
Approach and the
CDC Diabetes Prevention Program?

The Diabetes Prevention Program is a lifestyle program designed to reduce risk for type 2 diabetes through education, support, and habit tracking. It typically runs over 12 months, beginning with weekly group sessions for six months and transitioning to monthly sessions for the remainder of the year.

The program is highly education-focused, covering foundational topics such as nutrition, physical activity, stress management, and behavior change strategies. The curriculum is standardized, meaning participants receive the same content regardless of their personal routines, stressors, or lifestyle context.

Support is delivered in a group format, often with tracking tools such as food logs, activity tracking, and weight monitoring. For many people, this structure provides helpful accountability and a clear starting framework for change.

At the same time, sustained behavior change often depends on more than understanding information. Many participants enter these programs already familiar with much of the content, and the challenge becomes less about learning what to do and more about consistently applying it in real-world conditions over time. Retention rate is roughly 30-40%.

CDC-DPP Model

My work focuses specifically on the gap between knowing what to do and being able to do it consistently in daily life.

Most people I work with already understand the basics of supporting their metabolic health. What they often struggle with is not knowledge, but implementation—how to translate that knowledge into habits that hold up under real-life conditions like stress, time constraints, fatigue, shifting schedules, and competing priorities.

Instead of a standardized curriculum, this work is individualized and adaptive. We build around your life, your routines, your work schedule, and your environment, so the changes are realistic from the start rather than idealized.

We have nine focused meetings over three months, so outcomes can be tested and adjusted quickly. This allows us to see what is working sooner, refine what is not, and build momentum through consistency rather than delayed feedback.

My work integrates clinical nursing experience with training in behavior change and habit science, with a strong emphasis on translating health knowledge into sustainable action. When education is needed, I provide it in a clear and individualized way, making sure that information is directly applicable to you rather than a generic lesson.
